The template includes twelve 1-page curriculum assessment forms. Each form includes the following sections: Learning Outcome, Resources Required, Instructions for Developer, Written Assessment Format & Instructions for Students, and Performance-based Assessment Format & Instructions for Students. Each section includes instructions and recommendations.
